12 yr old boy acting up at school, then covering it up

Our 12 y/o son, who attends a small Christian school, was disciplined at school for crude jesters, at the lunch table in regards to a banana. In his meeting with principle, he was to write letter to us to tell us what happened and why he was being punished and a couple bible verses, etc. have us sign it and return it to the principle. Well, we were unaware of this because he forged dads initials and the principle suspected foul play and sent it to us. He is normally a good kid, nice, funny, enjoys school and participates, He does struggle turning in work on time, due to poor study habits. We have been dealing with a lot at home at well, I am personally undergoing treatment for breast cancer so that is consuming a lot of my husband and mine energy. But we are trying to juggle the kids homework etc as well. I think he didn't want to get into more trouble at home to cause me more stress. but at the same time he did lie to us and to the principle and we need to deliver consequences. Advice
