Potty training and perpetual frustration

Question

We used John’s process of potty training and I thought my daughter was potty trained (other than diapers for naps and bedtime). We started around 16-17 months old and now she is a few months past 2. I was quite happy with our success. She could use the toddler toilet by herself and pull her clothes up and down just fine. Every now and then she has an accident but it seems like it’s because she’s too busy playing to notice she needs to use the bathroom until it’s too late. For a while we were only dealing with one or two accidents every couple days and several days here and there would be accident free. But suddenly she began having lots of accidents. Today I have changed her outfit several times. Sometimes she just freezes and stares at the puddle forming around her feet instead of running to the toddler potty. I don’t know if these accidents are on purpose or because she’s too busy playing. What should I do? I’m tired of scrubbing carpet. I thought she was totally potty trained.
