Too many chances leads to deceit.

Question

My 15 year old asked if she could have Netflix. We told her we’d like to see her show us that she can manage her time better before we say yes. Gave her some thoughts on what we would like to see happen...ie: homework being done before late night, etc. She then took it off her Christmas list and said, “Never mind. You’re not going to get it for me anyway.” Come to find out, she got her friends password, got so into Netflix that she found herself freaking out about a school deadline she was going to miss. So. She lied, deceived, and broke the rule of no Netflix until we see an improvement. What should her punishment be? Should we write a behavior contract? Help!
