11 yr old Effort and Responsibility

Question

I am the Dad of an 11 year old (6th grade). She is not giving good effort at school. My wife and I have told her it is her responsibility and she needs to get help from the teachers, study nightly etc. Unfortunately, she also has some behavior problems at home....making family late in the morning for school and other events, arguing with parents, calling sister names, and sometimes ignoring parents (not in a angry way...just a lack of focus). My question is do we address her lack of effort at school (which is leading to declining/failing grades) or use the weekly chart to address home behavior? Or Both at same time?
