The "Terrible" Threes

Question

Our son just turned 3 last month and has typically always been well behaved anywhere in public and at the doctor etc. Lately he has been very difficult to comply when being examined at the doctor, dentist and even getting his haircut. He will cry, scream and flail in his body. He has never bitten, or hit anyone. I always prep him before hand when he has any appointment and he never shows any sign of fear but when we get there he clings to me. The last time he was sick and at the doctor he had to receive a nebulizer breathing treatment and even though it didn't hurt I suppose having to wear a mask and the loud noise of the machine may have scared him. We are just wondering if there's a better way to go about getting him to listen or if there are any ideas to calm him down and get him to comply?
