6 Year Old Not Motivated

Question

I have a 6 year old son with multiple medical issues. Among them is hydrocephalus, which often causes non-verbal learning disability and difficulty with executive function, but he does not have any true cognitive delay.

We are homeschooling currently, but I would like to put him in a small private school soon. Right now, we're trying to get him caught up with learning how to read. He has trouble following multi-step directions, is easily overwhelmed with simple tasks, and as a result of the medical issues and being "spoiled" by nurses when he's in the hospital (which is often), he prefers not to do things for himself. What is the best way to help him WANT to achieve, and to avoid needing an IEP in school?
