A pokey 12-year-old

My 12 year old daughter is very slow in preparing herself to leave the house. Even when we wake her up very early, we will still have to wait for her at the door while she scuttle about getting herself ready. We even run the risk of being late for our appointments. We usually wake her up one and a half hour before we have the leave the house. I think that should be enough time. We told her to pack her things to bring out, choose her clothes to wear and so on to ensure that she has as little to do as possible the next day. But the slow habit persist. If we rush her, she will panic and drop things and hyperventilate. How do we make her work faster and keep time?
